The transition from the hospital to skilled nursing wasn't as smooth as we all hoped. I don't know why we thought it would be seamless, but it has taken many visits and phone calls and conversations with doctors and nurses and staff to be sure Grandma has all of her medications and is receiving the care and assistance that she needs. Her blood pressure continues to be on the low side when she stands, so she is trying to eat well and work with OT and PT to get her strength back. There is an assessment meeting planned for tomorrow and hopefully we can stake out a course to get Grandma back to her apartment soon!
